    The game and practice of football is a full contact sport. Minor and major injuries are a common occurrence. The Banneker Junior Trojans (BJT), its officers, directors, coaches, etc. as well as any facilities or locations utilized by BJT cannot be held responsible or liable for said injuries in any way. Should injuries 'occur while participating in BJT activities, coaches will be allowed to make decisions concerning treatment of all injuries, minor and major, including transportation to medical facilities if deemed necessary, and should parents not be present at the time of such injuries.

    It is therefore fully understood and agreed by the undersigned parent(s)/guardian(s) and participant that the officers and directors of BJT, all coaches and assistant coaches of teams, are hereby forever released from any and all liability whatsoever because of injury, death, or bodily harm resulting from the act of practicing or playing football and any other and all activities involving BJT, including transportation.

    Furthermore, any officers, directors, coaches and assistant coaches of BJT, shall not be held liable for any decisions made concerning injuries. Any and all costs incurred as a result of medical treatment, procedures and/or transportation for injuries while involved in BJT activities shall be the sole responsibility of the players' parents.

    By signing this release, the signer also understands that the participant’s image may be duplicated by photograph, video or other means during the course of activity, and the participant grants permission for BJT to use participants image, including by publication.

    The signing of this agreement shall have the full force and effect of a "Covenant Not To Sue" as outlined by Georgia Law. All players are responsible for the safety of their football equipment. If a coach or BJT provided the football equipment used by a particular child, it is solely the child's parent/guardian's responsibility to assure the soundness of the equipment.
